Daily Routine

Morning

  • Medication administration (see medication list below)
  • Breakfast -- low-sodium, diabetic-friendly; ensure adequate protein
  • Morning walk with caregiver (15-30 minutes, weather permitting)

Afternoon

  • Light activity or rest as tolerated
  • Lunch -- monitor carbohydrate intake per dietary plan
  • Social time (reading, television, conversation with family or caregiver)

Evening

  • Dinner with Jess
  • Evening medication administration
  • Bedtime routine assistance if needed (mobility support, nightlight check)

Fall Prevention Protocol

  • Clear pathways at all times; no throw rugs, no cords across walkways
  • Nightlights in hallway, bathroom, and bedroom
  • Non-slip mats in bathroom (tub and beside toilet)
  • Grab bars installed in bathroom (tub entry, beside toilet)
  • Caregiver supervision during all transfers (bed to chair, chair to standing)
  • Encourage use of supportive footwear indoors (no bare feet, no socks without grip)
If a fall occurs: Assess the patient calmly. Do not move him if he reports pain. Call Jess immediately, then call 911 if there is any sign of head injury, inability to bear weight, or visible deformity.

Emergency Protocol

Chest Pain / Difficulty Breathing

  • Call 911 immediately
  • Then call Jess
  • Do not drive to hospital

Fall with Injury

  • Call Jess first
  • Assess -- do not move if pain
  • Call 911 if needed

Confusion / Behavior Change

  • Call Jess
  • Notify Dr. Emdur
  • Document what you observe

Non-Urgent Concern

  • Log in co-op.care app
  • Contact Jess at next opportunity
  • Note for next physician review

  • Log all care activities in the co-op.care app after each visit
  • Contact Jess for any concerns about daily routine or mood changes
  • Contact Dr. Emdur for clinical questions (medication side effects, new symptoms)
  • Encourage independence -- assist, do not replace. Let him do what he can safely do.
  • Note any changes in appetite, sleep, mood, or mobility in the daily log
